GLP-1 / glucagon dual agonist studied for weight loss and metabolic-associated fatty liver disease (MASH).
Survodutide is an investigational once-weekly dual agonist of the GLP-1 and glucagon receptors. The GLP-1 arm suppresses appetite while the glucagon arm raises energy expenditure and, notably, drives reduction of fat stored in the liver.
That liver effect is its standout feature: in a phase 2 trial for metabolic-associated steatohepatitis (MASH), a large majority of participants saw meaningful improvement in liver disease, alongside solid weight loss. It is being developed for both obesity and MASH.
As an investigational compound it lacks long-term safety data. The glucagon component can modestly raise heart rate, so slow titration matters.
Appetite suppression plus glucagon-driven energy expenditure.
Phase 2 obesity trial: strong dose-dependent weight loss
Substantial reduction in liver fat and improvement in steatohepatitis.
Phase 2 MASH: majority achieved histologic improvement

Evidence Profile
Investigational GLP-1/glucagon dual agonist with strong phase 2 results for both weight loss and liver disease (MASH).
Promising but early. Strong animal data and/or small/preliminary human studies, not yet confirmed in robust trials.
Controlled trials & clinical data in people
Positive phase 2 trials for obesity and MASH; phase 3 underway.
Preclinical results in animal models
Preclinical data supports the glucagon-driven energy-expenditure and liver-fat effects.
How plausibly it works at a biological level
GLP-1 / glucagon co-agonism is well-defined pharmacologically.
Depth of documented safety in humans
Class-typical GI effects plus a modest heart-rate increase from glucagon activity; long-term data pending.
Men with weight to lose who also have fatty liver concerns
Titrate slowly — the glucagon arm can raise resting heart rate. Investigational; long-term safety is not yet established.
Women targeting weight loss with metabolic/liver benefits
Avoid in pregnancy; use contraception during use. Emerging human data — proceed under guidance.

Glucagon activity can raise heart rate — titrate slowly.

The effects most commonly reported with Survodutide, when they tend to show up, and how to manage them.
Eat smaller, lower-fat meals; titrate slowly; it usually fades within 1–2 weeks at a stable dose.
Hydrate well, add fiber, stay active. Tell your clinician if severe or persistent.
Prioritize protein and don't under-eat — rapid loss can cost muscle. Lift weights.
Rotate sites; let alcohol dry fully before injecting.
Stop immediately and seek medical care — this is a red-flag symptom.
Individual responses vary. Stop and consult a clinician for severe, persistent, or unexpected reactions.
